Aspergilloma in an expand Chest Cavity

Aspergillus is a ubiquitous organism that is associated with a number of pulmonary syndrome [1-3] united of which is aspergilloma. [145] Aspergillomas are believed to conclusion from saprophytic colonication of preexisting, poorly drained intrapulmonary cavities. [146 Therapy is individualized and is generally restricted to refractory hemoptysis. [127-9] Treatment has included resection [12810] and systemic, [12] endobronchial, [7] or percutaneous [811] therapy with antifungal agents.

We quick in emergencies a unique case of an aspergilloma that formed within the cavity of an open-window thoracostomy. This allowed us to make direct observations regarding formation and resolution of an aspergilloma in vivo.

CASE REPORT

A 38-year-old male smoker at handed in October 1983 with gradual assault of pleurisy, chronic cough, shortness of breath, and daily febrile affections He was found to have an infiltrative mass upon the chest x-ray film. Transbronchial lung biopsies revealed a nonspecific organizing pneumonitis. Broad-spectrum antibiotics were instituted without notable clinical improvement. common month after removal of the chest tube, the patient came to our hospital with reaccumulation of fluid, persistent febrile diseases and malaise. His WBC enumerate was 6.0 X [10.sup.3]/cu mm (59 percent polymorphonuclear leukocyte 34 percent lymphocyte and 7 percent monocytes). The patient was HIV-antibody negative, had cutaneous hypersensitivity to mump antigen, and showed nonspecific elevations in serum immunoglobulins. A thoracocentesis revealed a yellow-green fluid with an LDH even of 5,357 IU, a grape-sugar level of 11 mg/dl, a protein plain of 7 g/dl, and a pH of 80; and a chest tube was inserted. A persistent air leak hinted the presence of a bronchopleural fistula. agricultures were again positive for M aviumintracellulare, which was single sensitive in vitro to rifampin and ethambutol.

Because of persistent feculent drainage, recurrent fevers, and malaise, an open-window thoracostomy was done. Four month later, despite improvement, a 30-cm grayish-black mass not radiographically apparent if it were not that adjacent to the bronchopleural fistula was visualized within the thoracostomy window (Fig 1) tillages and acid-fast stains from the cavity were negative. Fungal stains of the mass revealed mycelia of aspergillus, unless serum Aspergillus precipitins were negative. To inhibit fungal sprouting treatment was begun with nystatin tablets (100000 units USP) dissolved in physiologic saline solution and sprayed into the cavity three times daily. Antituberculosis therapy was discontinued. There has been marked improvement in the intracavitary mucosa (Fig 2) without return of fungus or mycobacteria.

DISCUSSION

Aspergillomas are hypothesized to progres by means of several developmental stages. [1] In the first, colonization of necrotic pulmonary tissue may meet the eye with subsequent formation of vegetative mycelia. In this first phase, radiographic and clinical signs are study to be absent. [1,4] In the secondary the aspergilloma is believed to be compos of the couple living and dead fungus, and it is presum to be radiographically apparent as a terminate of increased size. The fact that in comparison with prior chest x-ray films, a 3-cm mass was not radiographically apparent in our patient would recommend that factors other than size alone (ie, density of mycelia, calcification, etc) must be current within the mycotoma before it can be descryed by plain roentgenography. In the third stage the mycetoma is notion to soften, fragment, and unravel focal calcification. Our patient described the dislodgment of fragments for nearly a month prior to a 3-cm mass being incidentally detected

In our patient the aspergilloma could have occurr as a come of inhalation of spores with colonization of tissue adjacent to the bronchopleural fistula or through external contamination of the intrathoracic cavity. The fact that growing was localized to poorly healing tissue confimrs what has been described previously in surgical and postmortem studies. [1213] The remainder of the cavity, although chronically inflamed, did not appear necrotic, and no satellite fungal lesions were noted. That the aspergilloma grew in a well-drained cavity would glance at that poor tissue viability is perhaps more important than impaired drainage in fostering colonization and after growth of this saprophytic fungus in cavities.
