Tracheotomy is an ancient surgical ...

Tracheotomy is an ancient surgical management that was performed as early as 3000 years ago in Egypt and India. The diphtheria epidemics of the middle 19th hundred years widened the application of tracheotomy, which improved the consequence of patients with upper airway obstruction. posterior refinements of surgical techniques devised on Chevalier Jackson in the early 20th centenary convinced physicians of the utility of the course although attached risks and complications were well recognized and venerateed With the advent of mechanical ventilation, tracheotomy has now become the greatest in number commonly performed surgical procedure in patients hospitalized in critical care units. Despite its venerable history and widespread application, disputation still exists regarding patient selection and the indications for tracheotomy.

Major general indications for tracheotomy in the medical ICU are similar to other medical specialties, although the underlying illnesses may vary. These indications include relief of upper airway obstruction, assistance with pulmonary toilet, and airway bridle for long-term mechanical ventilation.

Patients with upper airway obstruction usually have neoplastic, infectious, or functional disorders that compromise ventilation. customary examples include nasopharyngeal tumors, epiglottis, Ludwig's angina, and bilateral vocal cord dysfunction. Patients are typically stabilized intially with translaryngeal intubation and subsequently regenerateed to tracheotomy if the airway obstruction is not remedial within 10-14 days.

Tracheotomy is also used in patients with poor bridle of airway secretions. Glottic dysfunction resulting from trauma or cerebrovascular disease may require tracheotomy not single to prevent aspiration of particulates if it be not that also to allow their removal through frequent airway suctioning.

The greatest in number common indication for tracheotomy in the medical ICU is in patients requiring long-term mechanical ventilation. Patients with neuromuscular disorders, chronic obstructive lung disease, edematous lung injury, multiorgan failure, and pneumonia who become ventilator pendent are frequently converted from translaryngeal intubation to tracheotomy to simplify their nursing care and improve their comfort. most numerous physicians agree that somewhere along the course of ventilator hanging patients tracheotomy is indicated. Major process in law exists, however, in determining the timing of the tracheotomy with disagreement center upon the relative risks and benefits of prolonging translaryngeal intubation v conversion to a tracheotomy. To assist in developing a rational approach to tracheotomy in the face of this polemics the benefits and risks of tracheotomy will be considered.

Tracheotomy improves the efficiency of airway suctioning in mechanically ventilated patients compared with translaryngeal endotracheal tubes. Although this benefit may not be clearly important in principally patients, instances of airway suppuration with copious secretions or airway hemorrhage warrant tracheotomy to obstruct airway obstruction and ventilatory compromise.

Patients undergoing long-term mechanical ventilation are more comfortable and mobile with tracheotomy tubes than translaryngeal tubes. This enhanced mobility allows patients relief from the ICU environment for short periods with excursions assisted on Ambu-bag ventilation. These intervals improve patient psyche and heighten motivation during long-term weaning efforts. Improved patient mobility and stability of the tracheotomy may also allow ventilated patients to be transferred from teh ICU to step-down intermediate care units.

Tracheotomy tubes are more securely fixed in the airway than translaryngeal endotracheal tubes, decreasing the incidence of accidental extubation of critically ill patients. In addition, should decannulation befall tracheotomy tubes with mature tracts are more quickly and safely replaced in patients with respiratory failure than nasal or oral endotracheal tubes in the absence of immediate personnel with clever intubation skills.

Tracheotomy avoids primary laryngeal injury although the act can aggravate subglottic inflammation initiated by way of prolonged translaryngeal intubation. Since mostly reports of laryngeal and subglottic stenosis after tracheotomy arise in patients with preceding protracted translaryngeal intubation, the occurrence of these complications do not sentence tracheotomy but promote its use early, before excessive laryngeal injury appears from nasal or oral intubation.

Tracheotomy clears the pharynx in mechanically ventilated patients promoting language and oral nutrition. Several techniques and devices exist to allow the patient with a tracheotomy to speak clearly, thereby assisting patients and family to cope with ICU stresse Additionally, patients who have neuromuscular disorders or chronic obstructive lung disease with respiratory failure may be able to eat after stabilization of their course, allowing removal of enteral or parenteral feeding lines.
