Gallium lung scanning is widely used to evaluate pulmonary inflammation in patients with interstitial lung disease unless has not previously been reported in crocidolite-exposed workers. In order to characterize the pulmonary inflammation caused at crocidolite inhalation, GLS and BAL findings were related to chest x-ray film changes graded according to the ILO classification of roentgenograms of pneumoconioses. In individuals with roentgenographic evidence of asbestosis (CXR [is greater than or equal to] 1/0 n = 15) 13 had a positive GL and 13 had an abnormal BAL. In asbestos-exposed individuals with equivocal chest x-ray film changes (CXR 0/1 n = 12) six had a positive GL and six had BAL changes (both GL and BAL abnormal in three) In individuals with a normal chest x-ray film (CXR 0/0 n = 8) sum of two units had a positive GLS and sum of two units BAL changes (both abnormal in 1) These data demonstrate that greatest in quantity subjects with crocidolite-induced asbestosis have an abnormal GL and BAL. In addition, many individuals with asbestos aspect and equivocal or no chest x-ray film changes have an abnormal GL and/or BAL, suggesting the nearness of active subclinical pulmonary inflammation in these individuals.

Subjects

Asbestos Workers

Thirty-five consecutive asbestos workers referr for investigation of possible asbestosis were studied with BAL and GL in addition to pulmonary function proofs All had been exerciseed in the Wittenoom crocidolite-mining industry in Western Australia. Methods

Clinical Assessment

All enslaves were assessed by one keeper (AWM) for the presence of crackles upon auscultation of the chest.[5]

Chest X-ray Examination

Routine PA chest films were obtained using a standard high kilovoltage technique with focus to film distance at six feet The films were graded on two independent, trained observers (AWM and BWR) and defined as demonstrating asbestosis if, based upon the ILO classification of roentgenograms of pneumocoinioses,[6] diffuse irregular opacities of grade 1/0 or greater were recorded. In three cases where the scores of the brace readers differed, the lower grade was recorded. The asbestos workers included 15 with asbestosis (defined by dint of chest x-ray film criteria) and 20 without film evidence of asbestosis of whom 12 had equivocal chest x-ray film abnormalities (grade 0/1) and 8 had a normal CXR (0/0)

Pulmonary Function

Forced expiratory mass in one second and FVC were measured by the agency of integration of the signal from a digital pneumotachograph. Gas transfer was measured using the single breath technique. Total lung capacity and its subdivisions were measured in a presence-compensated emanate body-plethysmograph. Predicted levels of lung function for [FEVsub1] FVC [FEVsub1]/FVC and TLC were obtained from Cotes[7] and TLco from Miller et al.[8]

BAL
